Sound Physicians Hospitalist Services Bringing Community Together to Address Transitional Care Needs

An educational event to connect providers about strategies for care coordination

    TACOMA, WA, November 04, 2016 /24-7PressRelease/ -- Sound Physicians announced today it is bringing together Bremerton, Washington area primary care physicians, hospital case management, Kitsap County Cross Continuum Care Transitions Project (KC4TP) and hospitalist teams to learn more about Transitional Care at an educational event on November 5. The educational event will be held at the recently renovated Bremerton Community Theatre where the play, 'Waiting in the Wings,' by Noel Coward, opens the day prior to the event.

Sound Physicians provides inpatient care through its 21 member hospitalist team supporting Harrison Medical Center in Bremerton and Silverdale, WA. Sound Physicians partnered with Harrison Medical Center starting in 2002, with a focus on improving quality, patient satisfaction and lowering the cost of care. The hospitalist team is led by Dr. Rana Tan, Chief Hospitalist, who has served on the Board of Trustees at Bremerton Community Theatre since 2006 and also co-stars in the 'Waiting in the Wings' production.

"Our hospitalist team has developed a network of services working closely with the community of primary care providers, hospital case management and KC4TP support patients after they leave the hospital to ensure they have the right care when they are discharged," said Devon Zoller, M.D., Associate Chief Medical Officer for the Sound Physicians Transitional Care team. "We are committed to drive improvement in clinical outcomes and ensure patients have the right care at the right time throughout the acute episode of care and as they transition from the hospital to their next site of care. This period, immediately after discharge, is especially critical for high-risk patients."

Sound Physicians' team of hospitalist providers, hospitalist nurses and transitional care nurses work with Harrison Medical Center's case managers, patients' primary care providers, and community team members including KC4TP to ensure patients receive the right care when they leave the hospital. Working together to assess and address patients' needs range from home health, medication management, patient education, skilled nursing, rehab, palliative care or hospice, depending on the individual patient's care requirements.
