Dr. Kikukawa has dedicated himself to the practice of psychiatry for many years, most recently serving with the Kyoto Min-iren Chuo Hospital in Japan. Hospitals often require psychiatrists on staff to treat patients who have emotional disorders, especially those recovering from grievous wounds or those who have received trauma to the head. Dr. Kikukawa's service into rehabilitation softens the blow and affords a valuable metric for physicians. In Japan, the number of elderly people suffering from dementia continues to grow. Accordingly, the ratios of elderly dementia patient in hospitals increase year by year, and a dementia specialist is required for an appropriate diagnosis and treatment of dementia more and more. He has contributed as a dementia specialist and instructor.

In 2000, Dr. Kikukawa went on to the Matsuyama Cardiovascular Medical Center, and began to serve as a staff psychiatrist for Nishi Ube Hospital. He became a staff psychiatrist for Ogori Makihara Hospital in 2005. While attending at Ogori Makihara Hospital, Dr. Kikukawa and his wife, Reiko Kikukawa, conducted a study on sodium valproate with seven patients in a convalescent rehabilitation ward. Using the Japan Stroke Scale- Emotional Disturbance Scale and the Barthel Index, they determined that sodium valproate yielded a marked improvement on the standards of a recovering patient's daily life. He was certified as a mental health psychiatrist by the Ministry of Health Labour and Welfare in Japan in 2008. He became a staff psychiatrist for Kyoto Min-iren Chuo Hospital in 2015. He engages in psychiatry generally (dementia and developmental psychiatry in particular). In addition to clinical psychiatry, Dr. Kikukawa also contributes to numerous professional articles and psychiatric organizations.
