Now professor emeritus of orthopedic surgery at the University of Pennsylvania, Dr. Smith began his career at the University in 1977 while completing his orthopaedic surgical resident training. He then began research and teaching as an assistant professor of Orthopaedic surgery, associate professor of Orthopaecdic surgery, ultimately being promoted to full professor of Orthopaedic surgery in 1996. Dr. Smith is perhaps best known for inventing and serving as director of the Pennsylvania Hip Improvement Program at the University of Pennsylvania School of Veterinary Medicine. This program, called PennHIP, is a radiographic method established to reduce the high frequency of canine hip dysplasia, a very common and painful osteoarthritic condition in dogs. Dr. Smith also had extensive experience in etiology and pathogenesis of hip osteoarthritis in dogs as a model for the disease in man. Dr. Smith was an early promoter of the "One Health" concept and has been a member of The Institute for Translational Medicine and Therapeutics (ITMAT) at Penn since its inception. Translational medicine focuses on developing new and safer medicines and medical procedures by supporting research at the interface of basic and clinical research.

First obtaining his undergraduate degree at the University of Pennsylvania in the school of engineering and applied science in 1970, Dr. Smith then completed a VMD in the school of veterinary medicine in 1974. He followed this opportunity with a postdoctoral fellowship at the Straumann Institute in Waldenburg, Switzerland, after which he was awarded a PhD in Materials Science Engineering at the University of Pennsylvania in 1982.

Dr. Smith has consulted as veterinarian and engineer for companies including Johnson and Johnson, Inc., in North Brunswick, NJ. Ultramet in Pacoima, CA, International Canine Genetics in Malvern, PA, Osteonics in Allendale, NJ, Ralston Purina in St. Louis, MO, among others. He served as President of the Veterinary Orthopedic Society, and is a longstanding member of the American Veterinary Medical Association, the New York Academy of Sciences, the Orthopedic Research Society, and the American Academy for the Advancement of Science.

Dr. Smith held leadership positions at the University of Pennsylvania School of Veterinary Medicine serving 8 years as Chief of Surgery and 7 years as Chairman of the Department of Clinical Studies.

Dr. Smith has authored more than 100 articles related to his work in engineering and veterinary medicine in journals that include the Journal of the American Veterinary Medical Association and Veterinary Surgery among many others. He also contributed book chapters to "Textbook of Small Animal Orthopedics," which was published in 1985 and to "Veterinary Surgery: Small Animal" published in 2017.

Dr. Smith has received many academic awards. Among them, in 1994, Dr. Smith was the recipient of a Research Excellence Award by Smith Kline Beacham. He received a 1996 Excellence in Research Award by the American Veterinary Medical Association. In 2004, Dr. Smith received the World Small Animal Veterinary Association Iams Saki Paatsama Award for Excellence in Orthopaedic Medicine and Surgery. He received the Blaine Award in 2011 for research by the British Small Animal Veterinary Association. Dr. Smith holds several patents relating to the Pennsylvania Hip Improvement Program at the University of Pennsylvania. Additionally, Dr. Smith in 1996 was included in the third edition of Who's Who in Science and Engineering for his work up to that date.
