Matthew Grissinger, RPh, FASCP, Director of Error Reporting Programs at the Institute for Safe Medication Practices (ISMP), will discuss barriers to optimal anticoagulation therapy and safety, common types of errors that occur with anticoagulants, and how human factors and practice issues can increase the risk of those errors among physicians, health-system pharmacists, nurses and patients.
An overview of the key elements in medication systems and results from ISMP's anticoagulation self-assessment will be presented to lay the foundation for in-depth discussion about the weaknesses in these key elements that lead to errors with the use of anticoagulants. Error prevention strategies, including improving order communication, storage, and administration of products, will be covered.
Objectives:
- Define latent failures and active failures and the role each plays when a medication error occurs.
- Identify system-based causes of medication errors associated with the use of anticoagulants and evaluate the potential ramifications and impact on organizations
- Describe strategies healthcare practitioners can use to prevent harm from the use of anticoagulant therapy.

About The Presenter: Matthew Grissinger, RPh, FASCP is the Director of Error Reporting Programs at the Institute for Safe Medication Practices (ISMP). His responsibilities include working with healthcare practitioners and institutions to provide education about medication errors and their prevention, review medication errors that have been voluntarily submitted by practitioners to a national ISMP Medication Errors Reporting Program (MERP) as well as serving as a clinical analyst for the Pennsylvania Patient Safety Reporting System (Pa-PSRS). He has extensive experience in long-term care, home care, and community pharmacy. Prior to joining ISMP, he served as a home care and long-term care pharmacy surveyor for the Joint Commission. Mr. Grissinger is a frequent speaker on pharmacy topics and current issues in medication safety. He has published numerous articles in the pharmacy literature, including regular columns in P&T, U.S. Pharmacist, and the PSA Patient Safety Advisory.
