For more than 15 years, Dr. Collazo has built a reputation as a globally recognized professional consultant, a distinction she did not initially anticipate when founding her private practice in Puerto Rico. Although her original intent was to serve local clients, her reputation for excellence quickly attracted organizations from the United States and internationally, including Singapore, South Korea, Argentina and Europe.

Among her most notable achievements are her disaster-related corrective action investigations, including her work following the Gulf oil disaster in Argentina and the Chevron explosion in Contra Costa County, San Francisco. In these cases, she focused on safety culture assessments and evaluating the effectiveness of corrective actions implemented several years after the incidents.

In FDA-regulated, GMP-critical environments, she has supported organizations operating under intense regulatory scrutiny, including those responding to Form 483 observations, Warning Letters, and multiple Consent Decrees. Her work focuses on conducting deep human-error and system-level investigations, assessing the true effectiveness of corrective and preventive actions (CAPAs), and addressing persistent compliance failures driven by cultural, procedural, and cognitive risk factors. These efforts are designed to meet FDA expectations for sustained remediation, executive accountability, and long-term operational control, rather than short-term corrective fixes.

Prior to her current role, Dr. Collazo served as director of technical training and human reliability at Bristol-Myers Squibb Company from 2006 to 2008. During this period, she oversaw technical training programs and human reliability initiatives that contributed to improved performance and reduced human-related errors. Her leadership supported the development of training systems aligned with operational excellence.

From 2005 to 2006, Dr. Collazo held the position of human performance reliability manager at Johnson & Johnson. In this role, she managed programs aimed at enhancing workforce reliability and minimizing human error across various operational processes. Between 2000 and 2004, Dr. Collazo was a training and organizational development manager at Schering-Plough Pharmaceuticals. She led efforts to design and implement comprehensive training strategies that improved employee performance and organizational effectiveness.

Dr. Collazo began her career as a training and organizational development supervisor at Pfizer Inc., where she worked from 1995 to 2000. Her responsibilities included supervising training initiatives and supporting organizational development projects that laid the foundation for her future expertise in human error reduction.

Education has been instrumental in shaping Dr. Collazo's distinguished career. She earned a Doctor of Philosophy in industrial-organizational psychology from InterAmerican University in Puerto Rico in 2008, following a master's degree in industrial-organizational psychology from the same institution. She also holds a bachelor's degree in general psychology from InterAmerican University.

Dr. Collazo's dedication to education is evident through her roles as dissertation reader and instructor at several universities, including Fielding Graduate University in California and InterAmerican University in Puerto Rico. She frequently serves as a substitute professor during visits to her alma mater, further advancing academic research in her field. Additionally, Dr. Collazo is the creator of multiple professional certification programs, including Human Reliability Expert Certification and Human Error Investigation and Root Cause Analysis methodologies.

Passionate about her field, Dr. Collazo's interest is deeply rooted in both personal curiosity and formative experiences during her education in Puerto Rico, a region heavily influenced by pharmaceutical manufacturing due to economic circumstances at the time. Her interest was further shaped by a pivotal encounter with a general manager who challenged her assumptions about the causes of human error, prompting her to develop a unique methodology for investigating such incidents.

Throughout her career, Dr. Collazo has demonstrated an unwavering commitment to giving back to the community. She has collaborated with regulatory agencies such as the United States Food and Drug Administration and Ireland's regulatory body, providing pro bono assistance during disaster-related corrective actions. She is currently organizing a charitable webinar dedicated to supporting homeless veterans and preventing veteran suicide, with all proceeds directed toward these causes.

In addition to her professional pursuits, Dr. Collazo enjoys exercising, reading, listening to music and studying new subjects. She is also the mother of one daughter who works in the medical field.
