SECAUCUS, NJ, February 12, 2019 /24-7PressRelease/ -- As over 45,000 professionals from across the world travel to HIMSS 2019 seeking the education, innovation, and collaboration needed to transform health through information and technology, Rx.Health is very excited to announce that the company has partnered with Healthwise, a leading creator of health content and patient education materials, to integrate Healthwise content into the RxStitch engine patient care pathways for targeted delivery to patients at the most critical time during their treatment. The content can be delivered to entire cohorts of patients using Rx.Health Bulk PrescribingTM tool with pre-built FHIR APIs and web-services to automate population health outreach by care managers in health plans and health systems.
"Getting the right health education to patients at the most relevant time during their care journey is a gap in offsite patient clinical support that Healthwise logically helps us fill," said Andy Pfau, Chief Operating Officer of Rx.Health. "Every provider can attest to the inefficacy of handwritten or paper instructions, which patients often lose or misplace within hours of their clinical visit. By providing the appropriate content or education materials within an Rx.Stitch care pathway, we're now not only providing content when a patient needs it the most, but also encouraging shared decision making tools that promote positive patient behavior. This is how we optimize patient education materials and modernize their use for value-based care."
The RxStitchTM care pathways take seemingly disparate digital assets, including health content and patient education materials, and combines them into a complimentary suite of bundled services that enhance patient care outside of the clinical setting. The appropriate tools are then introduced to patients and the most relevant points of care, ensuring high adoption and utilization of those materials.
"By partnering with Rx.Health we extend the reach of our rich multimedia content through a plug-and-play module in the EHR, care management systems, and patient portals, as well as in disease-specific health bots and digital care plans," stated Dave Mink, the Chief Client Officer for Healthwise. "We're excited about our partnership with Rx.Health, and the impact we can make by getting the right materials in front of a patient when they need it the most."
