After scheduling your exam you can expect an email from us containing a set of electronic intake forms. These forms must be completed at least 24 hours prior to your appointment. If these forms are not completed your exam may be cancelled by our office and a fee assessed to your account. Please see our Billing Policy for more information. You will need to upload a copy of your medical records and your Florida drivers license or State ID card. If you do not have your medical records on hand or do not have access to them, our records department can request them for you.

During your exam, your physician will answer any questions you may have, review your medical records and determine whether or not you qualify for a recommendation. If you do in fact qualify, your physician will then enter your into the Florida Medical Marijuana Use Registry. After being entered into the Registry you will receive an email with step-by-step instructions on how to complete your application and pay the $75 state-required annual fee. The application approval process takes approximately 7-21 business days. You will be notified via email when your application is approved and will receive a temporary Registry ID Card that can be used at any Florida MMTC until your physical card is received.
