He is also a key member of the governing board for both his practice and its affiliated surgical center, holding partial ownership and playing a pivotal role in all strategic decisions.

Alongside his work at Piefer Bone & Joint, Dr. Piefer has been a medical executive committee member at Memorial Hermann The Woodlands Medical Center since 2013, contributing to the hospital's operational oversight and quality initiatives. Additionally, he contributes as an orthopedic surgeon, speaker, board of directors member and partner at The Woodlands Sports Medicine Centre, where he has practiced since 2011.

In each of his roles, Dr. Piefer treats a wide range of sports medicine cases, including injuries related to rotator cuffs, anterior cruciate ligaments and meniscus tears. He performs complex shoulder, hip and knee replacements and provides emergency orthopedic coverage at Memorial Hermann The Woodlands Medical Centre, a level II trauma center, five days each month.

Throughout his tenure at Memorial Hermann, Dr. Piefer also served as chief of staff from 2023 to 2024 and chaired the Department of Orthopedics from 2021 to 2023 as well as from 2015 to 2019. Earlier in his career, he completed an orthopedic sports medicine fellowship at Taos Orthopaedic Institute in 2011 following his orthopedic surgery residency at the University of Alabama at Birmingham in 2010.

Over the course of 15 years, Dr. Piefer has been recognized for building cohesive teams dedicated to delivering the highest quality of patient care — a professional achievement he regards as his most meaningful. By fostering collaboration among his colleagues, he ensures that patients receive exceptional service in environments that remain conducive to both healing and professional fulfillment. Hailing from an equally accomplished academic foundation, Dr. Piefer began his journey with a Bachelor of Science in biomedical science from Texas A&M University, where he graduated summa cum laude in 2001 and was named an honors fellow. He went on to earn a Doctor of Medicine from Baylor College of Medicine in 2005, where he was recognized as the outstanding student in orthopedic surgery. His educational experiences were pivotal in shaping his approach to medicine; inspired by his father's career as a family practice doctor and influenced by his own athletic background, he found his calling in orthopedics after initially considering a career in business.

Underscoring his dedication to maintaining the highest standards of clinical practice, Dr. Piefer was board-certified as an orthopedic surgeon by the American Board of Orthopaedic Surgery in 2013.

Beyond his clinical responsibilities, Dr. Piefer has contributed significantly to civic organizations and educational initiatives. In 2017, he served as the medical team lead for St. Anthony of Padua Medical Mission, providing essential health care services to underserved communities in Honduras. He also directed St. Anthony's Adoration, Community, Theology and Service (ACTS) Retreat from 2018 to 2019 and was a member of the board of trustees for Frassati Catholic High School from 2016 to 2024.

Also known for his scholarly activities and public outreach, Dr. Piefer authored two published articles on anterior cruciate ligament reconstructions for Arthroscopy: The Journal of Arthroscopic & Related Surgery in 2012 and serves as a speaker for Orthopedic organizations. His wide-ranging public engagements include upcoming presentations at the Spring Conference for the Association of periOperative Registered Nurses (AORN), health care business lectures at Texas A&M University and participation on the Aspire Tour.
