The technology's rapid onset by bypassing first-pass metabolism and avoiding delays associated with traditional oral dosing has set it apart in the industry.

Orcosa operates with a dedicated team from its New Jersey facility, handling all research, development and clinical supply in-house. The company's research and development efforts extend to pediatric formulations and treatments for diseases prevalent in regions with limited access to clean water. Mr. Mileto's expertise as a scientist and operations leader ensures a focus on quality and manufacturing, while his partners manage regulatory affairs and intellectual property strategy.

Mr. Mileto was the vice president and general manager at Concours Pharmaceuticals from 2018 to 2020. He previously spent 2016 to 2018 as the director of external manufacturing and quality at NewMarket Pharmaceuticals. Among other roles, Mr. Mileto was a technical services leader at Catalent Inc., a leading global contract development and manufacturing organization, in 2016, after previously serving as an operations manager for advanced delivery technologies and senior auditor for quality assurance operations, dating back to 2007.

Mr. Mileto began his career as a lead auditor at Sandoz Group AG from 2005 to 2006, focusing on quality systems within pharmaceutical manufacturing. He then joined Cardinal Health as a quality assurance auditor from 2006 to 2007.

Mr. Mileto earned an Associate of Arts in liberal arts from Ocean County College in Toms River, New Jersey, in 1996, followed by a Bachelor of Science in biology and chemistry from Monmouth University in West Long Branch, New Jersey, in 1999. These academic achievements provided him with a strong scientific background that has been instrumental throughout his career. Additionally, Mr. Mileto holds credentials as a Certified Pharmaceutical GMP Professional from the American Society for Quality.

Throughout his career, Mr. Mileto has been an active member of professional organizations, including the American Society for Quality and the Parenteral Drug Association. Beyond his professional pursuits, he is committed to civic engagement through volunteer service with booster clubs supporting youth activities.

Among Mr. Mileto's most significant accomplishments was leading the development of Orcosa's groundbreaking buccal tablet technology. Mr. Mileto began work on the technology in 2018 and led its advancement through early formulation and testing. By 2020, Mr. Mileto supported its submission for initial clinical trials with the U.S. Food and Drug Administration during a particularly challenging period marked by the COVID-19 pandemic. Remarkably, Orcosa moved from product development to dosing its first human participant in just over 18 months, a rare achievement in the industry. The initial trial involved 100 participants undergoing arthroscopic rotator cuff repair surgery, one of the most painful procedures of its kind. Results demonstrated statistically significant pain reduction on days one and two post-surgery, increased patient satisfaction and reduced opioid usage compared to standard care, and results were presented at several professional conferences and published in the American Journal of Sports Medicine.

Mr. Mileto is also credited as both lead inventor on Orcosa's first US patent and co-inventor on their second patent.

In light of his contributions within the industry. Mr. Mileto presented at the BioNJ conference for the pharmaceutical sector, then joined his chief executive officer for an NJBIZ podcast after connecting with a reporter at BioNJ. He was also named to NJBIZ's 2025 People to Watch in Healthcare, a distinction highlighting his impact on health care innovation.

Looking ahead, Mr. Mileto aims to build a successful company that delivers transformative products to market, improving lives across pediatric populations and broader communities alike, while also expanding operations into a full-scale manufacturing facility in New Jersey that employs up to 150 individuals.
